Welcome to Forma Fitness Studio. Our Terms and Conditions set out the guidelines for memberships, class bookings, cancellations, and studio policies. By booking a class or becoming a member, you are agreeing to these terms, which are designed to ensure a safe, respectful, and supportive environment for everyone.

Forma Fitness Studio | Porto and Aveiro
This document governs the purchase of classes and memberships and the use of Forma Fitness Studio. Current class schedules, opening hours and prices can be found through the channels listed below. Clients must read these conditions before attending the studio.
1 Scope and access:
The Porto and Aveiro studios are operated by Forma Fitness Studio, Lda., corporate identification number (NIPC) 518605639, with its registered office at Avenida Europa, no. 712, 3800-012 Aveiro. These conditions apply to everyone who uses either studio.
Access to classes is restricted to registered clients with a valid booking or people authorized by the studio. The minimum age for participating in classes is 14. Before a minor’s first class, their legal representative must sign the liability declaration available at reception.
The studio does not provide childcare. Children who are not participating in a class may not remain at reception or elsewhere in the studio while the person accompanying them attends a class.
2 Classes, schedules and bookings:
Forma offers small group fitness classes. Class descriptions and availability are listed on the Forma website and app. Opening hours are displayed at each studio and published on the website. The current class schedule is available on the website and app and may be changed at any time for operational reasons.
Classes require advance booking through the website or app, subject to availability. Machine selection is part of the booking process and follows the order in which clients book the class. Private or individual classes, when offered, require advance booking and are subject to availability.
Passes and memberships are personal and may not be shared. The usage period, credit balance and expiration date are shown in the client’s account or the Forma app. Clients are responsible for monitoring these dates and using their credits before their pass expires or their membership month ends. Booking a class does not extend the usage period; the class must take place while the pass or membership credit is valid.
Unused membership credits do not carry over to the following month. VIP memberships are an exception, as their credits may be used at any time throughout the full term and validity period of the VIP membership.
3 Class cancellations and missed classes:
To avoid losing a credit, bookings must be canceled at least 10 hours before the class begins. For clients with a 3-month VIP plan, the cancellation window is 5 hours. For clients with a 6- or 12-month VIP plan, it is 2 hours.
Missing a class or canceling outside the permitted window results in the loss of one class credit from a pass or membership with credits. A paid single class is considered used. Cancellations must be completed through the app or website.
Arriving more than 3 minutes after the class begins also results in the loss of the class credit.
If a technical issue prevents cancellation through the app or website, the client must contact the studio immediately by phone, message or email and explain the situation.
4 Arrival and lateness:
Clients should arrive 5 to 10 minutes before each class. For the first class, arrival 20 minutes in advance is mandatory to become familiar with the machine and receive instructions on how it works, meet the instructor and receive an introduction to the class. Clients who arrive for their first class after it has begun may not enter and must reschedule for a time when they can arrive 20 minutes in advance.
For subsequent classes, entry is not permitted more than 3 minutes after the class begins, as late entry may distract the instructor and other clients. The credit is lost in accordance with the missed-class policy.
5 Waitlist:
Clients on the waitlist are notified through the app, by email or by message when a space becomes available and they are added to the class. They must keep app notifications enabled and remove themselves from the waitlist if they can no longer attend. If they are added within the late cancellation window, the usual cancellation rules apply.
The waitlist closes 1 hour and 45 minutes before the class begins. After this time, anyone wishing to attend should monitor the booking page and book directly if a space becomes available.
6 Membership renewal, cancellation and pauses:
Memberships renew automatically every month using the payment method provided when the subscription was created. To cancel a membership, the client must notify the studio in writing at least 15 days before the next billing date. This notification may be sent to the relevant studio’s email address.
A membership without a commitment period cannot be paused for a personal absence. A 12-month contract that includes this benefit may be paused once during the contract for a maximum of one month, upon written request. The dates and the effect of the pause on billing and the contract end date must be agreed with the studio in writing.
7 Prices, payments and failed payments:
Current prices are available at reception, on the website and in the app. Payment is due when purchasing a class or pass, or on the membership billing date. Forma accepts the electronic payment methods available at the time of purchase, including debit or credit cards and MB WAY when available. Memberships may also be paid by direct debit. Cash payments are not accepted.
Membership renewals are charged using the payment method provided when the subscription was created. Clients wishing to change this payment method must do so at least 15 days before the next billing date.
If a renewal payment fails, all future bookings are automatically canceled. After completing payment, the client must book their classes again, subject to availability. While payment remains outstanding, classes cannot be booked or attended.
8 Changes and cancellations by the studio:
The studio may temporarily close or change classes due to training, illness, maintenance, force majeure or other operational needs. Whenever possible, the studio will communicate changes in advance and seek to reschedule affected classes. If Forma cancels a booked class, the credit is returned to the client’s account.
9 Health and safe participation:
Before class, clients must inform the instructor of any injury, pregnancy, health problem or other condition relevant to their safe participation. Clients should not attend classes when they are ill or have symptoms of a contagious illness. Anyone who feels unwell during class must stop exercising immediately and inform the instructor.
In the event of injury or pregnancy, a medical certificate confirming fitness to participate in the exercise activities offered by Forma, including any restrictions, must be provided before participating. Without this certificate, the client may not attend classes. Instructors adapt exercises to the client’s circumstances when appropriate.
Instructors may make postural corrections through physical contact. Clients who do not wish to be touched must inform the instructor before class and may also decline a correction during class.
10 Studio use, hygiene and conduct:
Grip socks and appropriate exercise clothing are mandatory. Participation is not permitted without grip socks; socks are available to purchase at the studio. A personal towel is recommended but is not mandatory. Clients should avoid applying excessive amounts of lotion or cream before class and must not use products containing glitter or shimmering particles, as these can affect grip and make the machines more difficult to clean. Machines are sanitized after every class.
Clients must follow the instructor’s instructions and use machines and equipment correctly. All cell phones, including those left in the personal belongings storage area, must be set to silent before class. Cell phones must be stored away and may not be used during exercise. Photos and videos may be taken before or after class. Filming your own participation during a class requires permission from the manager or instructor. Care must be taken not to film other clients nearby.
The studio may refuse access in cases of outstanding debts, disruptive behavior, threats to safety, serious breaches of hygiene rules or other serious violations of these regulations. Clients must respect the team, other clients and the facilities.
11 Insurance, personal belongings and video surveillance:
Forma has liability insurance covering clients under the terms of the relevant policy. Information about the insurance is displayed at the studio. Clients must report any accident to the team immediately.
Clients are advised not to bring valuables to the studio. They are responsible for their personal belongings and should use the designated storage area. Lost items should be reported to reception. Video surveillance is intended to protect people and property, including studio equipment. Cameras cover reception, the personal belongings storage area and the training room. Forma is not responsible for loss of or damage to personal belongings, except where it is legally liable.
12 Privacy, complaints and applicable law:
Forma processes personal data in accordance with its Privacy Policy, available on the website. Clients may request access to, correction of or deletion of their data and may object to marketing communications. For privacy requests, questions, suggestions or complaints, the contact address is info@formafitnesspt.com. A Complaints Book is available at each studio.
These conditions are governed by Portuguese law. The current version is available on the Forma website and in the client’s account and is displayed at the studio. Changes to the conditions are published in these locations.
13 Refunds:
A client’s decision not to use a purchased service or inability to attend does not automatically entitle them to a refund. Refunds and other rights provided for under applicable law remain unaffected.
Acknowledgment:
By making a purchase and attending classes at Forma, the client confirms that they have read and understood these Terms and Conditions and Internal Regulations.
